What happens to my hospital accumulations when an NDC is changed?

Entities are expected to replenish with the same National Drug Code (NDC) that was dispensed. As a rule, 340B Accumulations are non-transferrable. However, manufacturers can create new NDCs that don't match the prior NDC.

As soon as you buy the new NDC, it will be available to match in the crosswalk.

The accumulations on the old NDC are "lost" unless you contact the manufacturer to get permission to move the accumulations from the old NDC to the new NDC.

TIP: Don't forget to document the approval and the accumulator adjustment.
